Only refer if the patient is older than 50 years

Do the following investigations: Treat any infection:

  • DRE
  • PSA
  • MSU
  • Semen culture

If the patient is younger than 50 years, investigation and treatments are rarely worthwhile.

Prostate cancer is the only significant condition that needs excluding, even though this is a rare presentation.

Trial finasteride if haematospermia persists. Finasteride is available on special authority for symptomatic Benign Prostatic Hypertrophy if non-selective alpha blockers are ineffective, not tolerated by the patient, or contraindicated.
