Crisis Team Communication

We have had some feedback around communication with Primary Care following a crisis assessment. As a result there will be a trial of a new format of informing the GP when they have been seen by the Crisis team. This can following a self-referral , through ED or after a GP has referred them. You will now receive a one page summary of this assessment which should include all the pertinent and important details of that encounter ie presenting issues, medication/ treatment, plans for follow  up and key workers involved.

The aim is to make this relevant, timely and short! If more detail is wanted then the full and complete assessment document can be requested.
