The hospital is currently initiating a project to look at how we can better support people who are frequent attenders at the hospital. Previously ED has developed management plans to improve their care when in the department but there has not been an integrated approach considering the whole patient journey; before the patient attends ED and then when they leave, either back home or into an inpatient bed.
Most general practices will be aware of their patients who attend hospital frequently. We want to approach this from the perspective not that this attendance is inappropriate, but rather is there a better way of delivering the care that the patient needs.
If you have a patient that you believe could be managed differently please contact Dr Alastair Maclean, one of the ED consultants, by email ( Alastair.Maclean@bopdhb.govt.nz ). The aim is to develop integrated management plans for patients that link up general practice, ED, specialist teams and other community services.
This is a new process and so if flooded with patients we might not be able to do all at once, but it would be good to know if there is an appetite to work in this collaborative manner in general practice.
