Palliative Care in Heart Failure

Determining the onset of terminal decline in heart failure patients can be more difficult than in cancer patients:

 

Clues to the palliative phase in heart failure include:

  • Worsening/ refractory symptoms (NYHA Class IV).
  • Requirement to withdraw prognostic medication due to refractory hypotension.
  • Worsening hyponatraemia.
  • Cardiac cachexia.



Implantable Cardiac Defibrillators (ICDs):

  • Consider discussion with patient and family regarding switching off.
  • These can be inactivated at Pacing Clinic - contact phone 07 5798291.

 

The Heart Failure clinic (nurse-led and accessed by the cardiologists) can be involved to optimise medications and monitor fluid state.
