Future Care Planning training

How to better support your patients at the end of life

The BOPDHB Future Care Planning Advisory Group will offer fully funded positions on this training, which will be on 14-15 November 2016 with the venue to be confirmed at a later date.

Applications for funded training are made via the survey monkey embedded in this flyer; please note there is a prerequisite of completing the eLearning Level 1 ACP training which you can access at www.advancecareplanning.org.nz.

There is a field that requires you to enter the details regarding invoicing – please put my contact details, c/o the Future Care Planning Advisory Group into this field.
